Learning disabilities - community and inpatient services
The learning disabilities services provide community and inpatient support to adults with learning disabilities across the Bradford district.
There are approximately 2,500 adults with learning disabilities in the district. However, some of these people have more complex needs and require additional support from specialist clinicians to ensure they can stay as healthy and well as possible. This support may meet either physical needs or mental health needs, which is provided by the Bradford District Care NHS Foundation Trust (BDCFT).
The health support team
The health support team is a multi disciplinary team of health clinicians that includes nurses, physiotherapists, occupational therapist, speech and language therapist, dieticians, psychologists, psychiatrists and health support workers. The team help with a variety of health needs including assessments for transition, mobility, behavioural needs, sensory and environmental issues, swallowing, communication and more.
The team also work closely with primary and secondary health care providers to ensure services are accessible.
BDCFT have specialist dental, podiatry, ophthalmology and audiology clinics for people with learning disabilities who are unable to access mainstream health services.
Assessment and treatment unit (inpatient services)
Lynfield Mount Hospital has a specialist learning disabilities ward. The ward supports people with learning disabilities who may also have mental health needs or behavioural issues. The majority of people would access this service following a mental health assessment.
Annual health check
People with learning disabilities aged 14 and over can have a free yearly health check at their GP surgery. It takes around 30 minutes and makes sure you are feeling well and keeping healthy.
You can contact the service on 01274 497121.
